Grace Hong, APRN, is a nurse practitioner in the Division of Infectious Diseases and Immunology where she specializes in COVID-19 (including management of COVID-19 Long Haul), immune system deficiencies, HIV, travel, and post-hospital follow-up care. She has extensive experience in hospitalist care, including pediatric hospitalist medicine and specialty consultative services. She was an active leader during the COVID-19 pandemic, consulting in all hospital-wide COVID-19 task forces and ensuring all guidelines and protocols provided the best possible care for our patients, families, and team members. She also sees patients in our outpatient clinic.
Grace is an active member of the Antimicrobial Stewardship Program, senior development specialist for the Clinical Effectiveness Program, co-chair of the Clinical Effectiveness Committee, and co-chair of the CLASP Committee. The Clinical Effectiveness Program aims to improve the quality and value of care that pediatric patients receive in primary care, ambulatory, and acute care settings across the state of Connecticut and other nearby regions. It is comprised of the Clinical Pathways Program (which encompasses all hospital-wide clinical pathways, educational modules, and quality metrics), and the CLASP/Referral Guidelines Program (which encompasses referral guidelines for primary care providers). Grace is the co-author and content expert of Connecticut Children’s Community-Acquired Pneumonia Pathway, Fever and Sepsis Evaluation in Neonates (0-28 days) Pathway, HIV nPEP Pathway, Skin and Soft Tissue Infection Pathway, Animal Bite Skin and Soft Tissue Infection Pathway, as well as numerous Infectious Disease Outpatient Management Pathways and primary care referral guidelines.
Grace received her Black Belt Lean Certification in 2022 and is a coach for Connecticut Children’s Organizational Excellence. She strives to provide safe, reliable, and effective patient care by utilizing principle-based approaches to continual improvement and implementing sustainable change within the institution.
